Students entering PreK, Kindergarten, 6th and 9th grades must have a physical exam dated within 1 year.  A sports physical does not count for these grades.

Students entering Kindergarten, 2nd, 6th and 9th grades are required to have a dental exam on file by May 15th of the current school year.  Dental exams must be dated within 18 months of that date.

All Kindergarten students are required to have an eye exam completed by an optometrist or ophthalmologist upon entry of Kindergarten.

This form should be completed if a student is going to take any prescription or non-presciption medication at school.
