All medications including prescribed and over-the-counter must be brought in by an adult in the original packaging, with the student’s name, medication name and dosage. Students are not permitted to transport medications to or from school except for inhalers and epinephrine.
Students will not be given prescription and/or over-the-counter medication without a medication authorization form completed by the student's provider and signed by the parent/guardian.
A medication authorization form for an inhaler must be completed by the provider and signed by the parent. A copy of the prescription label for the inhaler may be used for the provider's portion of the form. If the student carries their inhaler on themselves, the self-carry and self-administration sections of the medication authorization form must be completed by the parent/guardian.
An asthma action plan is required for students with asthma and it is completed by the student's provider and signed by the parent/guardian.
A medication authorization form for epinephrine (EpiPen or nasal spray) must be completed by the provider and signed by the parent. If the student carries their epinephrine on themselves, the self-carry and self-administration sections of the medication authorization form must be completed by the parent/guardian.
An allergy action plan should be completed by the student's provider and signed by the parent/guardian.
